House Bill 914 of the 79th Legislature of the State of Texas added Chapter 176 to the Local Government Code.  Chapter 176 requires vendors and other persons to file a Conflict of Interest Questionnaire, Form CIQ, if the person “contracts or seeks to contract for the sale or purchase of property, goods, or services with a local government entity; or is an agent of a person in the person’s business with the local governmental entity.”

 

Effective January 1, 2006, a person that contracts or seeks to contract for the sale or purchase of property, goods, or services with a local government entity must timely file a conflict of interest questionnaire with the records administrator of the school district (in La Grange this is the Assistant Superintendent for Finance) no later than the 7th business day after the vendor begins contract discussions or negotiations with the school district or submits to the district an application, response to a request for proposals or bids, correspondence, or another writing related to a potential agreement with a school district.

 

Following is the Conflict of Interest Questionnaire, Form CIQ, which is to be completed describing any affiliation or business relationship with any La Grange ISD Board Member, officer, or employee having involvement in purchasing goods or services.  If there are none, state so in the spaces provided. 

           

 

LOCAL GOVERNMENT OFFICER CONFLICT DISCLOSURE STATEMENT (Form CIS):

Chapter 176 also requires certain local government officers to file a Conflict Disclosure Statement, Form CIS, with the records administrator of a school district if

  1. The school district has contracted with a person or is considering doing business with the person and
  2. The superintendent or board member or a family member within the first degree or consanguinity or affinity either has an employment or other business relationship with the person and receives taxable income or
  3. Has been given by a person one or more gifts that have an aggregate value of more than $250 in a 12-month period preceding the date the local government officer became aware of the facts that require the filing of a conflicts disclosure statement.

Following is the Conflicts Disclosure Statement, Form CIS, which must be filed with the records administrator no later than 5 p.m. on the 7th day after the date the officer becomes aware of the facts that require the filing of a statement.
